Within the first 24 hours of hospitalisation

BUN >25mg/dL (8.9 mmol/L)

Impaired mental status (Glasgow coma score <15)

Evidence of SIRS (Severe Inflammatory Response Syndrome)
(any 2 of the 4 criteria below)

  • Temp >38°C (100.4°F) or <36°C (96.8°F)
  • Heart rate >90/minute
  • Respiratory rate >20/minute or PaCO2 <32 mmHg (4.3 kPa)
  • WBC >12,000/µL, <4,000/µL, or >10% immature (band) forms

Age >60 years

Pleural effusion

Ranson's Criteria

At admission

Age >55 years

WBC >16,000/µL

Glucose >11.1 mmol/L (>200 mg/dL)

LDH >350 IU/L

AST >250 IU/L

During initial 48 hours

Haematocrit decrease >10%

BUN increase >1.8 mmol/L (>5mg/dL)

Calcium <2 mmol/L (<8 mg/dL)

PaO2 <60 mmHg

Base deficit >4 mEq/L

Fluid sequestration >6 L
